Very Important   This report goes to the General Conference in a few days. Please fill it out completely and send it promptly. Thank you. Observations

1.	Any delay or omission causes trouble in the preparation of the report the Conference has to send to the union. It is very necessary that you send this report, even though members have not been received or dropped. Every time a report fails to come in, or some data is missing on it, it causes disagreement in the statistics of the Organization.
2.	In relation to baptisms from another faith, please indicate the church in the section marked, "Church From Which Person Came." This section also to be used in relation to transfer by letter within Sisterhood of Adventist Churches.
